Real Discipleship Survey
This tool can help you get a sense of what stage of growth you may be on for each of the six dimensions. The survey can be taken as often as you want, although once a year is recommended. Each time you take the survey, it will produce a report with suggested studies and activities that will help you move to the next stage of growth in each dimension.
The survey consists of four statements contained in each of their own colored blocks for each dimension. For each of the six colors (one color for each dimension), choose the statement that best describes where you believe you are in your journey. If you need additional assistance, there is a “Tutorial” button that will take you to instructions for the survey. If you wish to change an answer, the “Reset” button will reset the survey. When you have chosen a statement for each of the six colors, you can select “submit survey” button.
Follow the prompts on your screen to return to your dashboard. Each time you complete the survey, your past surveys will also be listed showing the date of completion. You can view the “Results” on the screen or download a “PDF” to allow you to print it.
No one else can see your answers. We can pull a report that shows a matrix of how many people are in each stage of growth for each dimension, but not who they are. This information will help shape some of the studies we may select and some of the sermon series that are preached to meet the overall need of the congregation.
Instructions for Individuals
To login to your dashboard:
1. If you have not registered an account with us first register:
a. Visit: https://www.discipleshipsurvey.com/1/
b. Click the large blue button that says "Use Your Church's Access Code" in the middle of page
c. Enter CTL7D as the “conference code”
d. Click "Submit", then “Continue".
e. Select your Church (Grace).
f. Enter your name.
g. Enter your email.
h. Enter a new personal password.
i. Select your language.
j. Click “Join” - This will create an account for you and give you a message that your membership has been added.
k. Click "Login Here" to proceed to the login page.
2. If you have already registered an account as detailed above login:
a. Visit: https://www.discipleshipsurvey.com/1/member_login.php
b. Enter your email and password.
c. Click "Submit".
How use your dashboard:
1. YOU MUST CHOOSE to identify with specific group(s) before taking the survey. Please choose all that apply.
a. Select the second blue button labeled “Edit Profile” to choose your groups.
b. Leadership - Select “YES" if you are serving on staff, any of our church committees or as a small group facilitator.
c. Worship - Select “YES” for the worship service you typically attend (choose both if you regularly attend both).
d. Small Groups - Select “YES” for the small group time you are participating in.
e. Click "Update" to save changes.
f. Once you have saved your groups, click “Return"
Take the Survey:
a. Clicking "Take Survey" will begin the survey process.
b. When taking the survey, the blue button in the upper right "Tutorial" will assist with instructions for taking the survey.
Previous survey results will be available on the dashboard, sorted by date taken. You can view the results, or make a PDF.
